چکیده

P LASMIN, a proteolytic and fibrinolytic enzyme of blood, is present normally as an inactive precursor, plasminogen, in the circulating blood. In various pathologic conditions plasminogen may convert into active plasmin in vivo. Recently it has been reported by Okamoto et al. that e-aminocaproic acid (ACA ) has a potent inhibitory activity in the dissolution of fibrin by plasmin when activated by streptokinase ( 5K) in plasma. It is one of the purposes of this communication to determine the effect of this compound upon the fibrinolytic process. The present report is concerned particularly with re-examination of the theories which have been proposed to explain the mechanism of fibrinolysis. Evidence obtained here indicates that certain newly mentioned products, metafibrin and fibrinolysopeptide, may be split off from fibrin in the early phase of the fibrinolytic process.

Epsilon-aminocaproic acid for treatment of fibrinolysis during liver transplantation.

In 97 adult patients receiving liver transplants, the coagulation system was monitored by thrombelastography and by coagulation profile including PT; aPTT; platelet count; level of factors I, II, V, VII, VIII, IX, X, XI, and XII; fibrin degradation products; ethanol gel test; protamine gel test; and euglobulin lysis time. Preoperatively, fibrinolysis defined as a whole blood clot lysis index of...

Epsilon-aminocaproic acid in the treatment of patients with acute promyelocytic leukemia and acquired alpha-2-plasmin inhibitor deficiency.

Patients with acute promyelocytic leukemia often develop bleeding diatheses during treatment. In seven patients who had this disease, the plasma level of alpha-2-plasmin inhibitor was the best predictor of severity of coagulopathy and bleeding. Clinical bleeding occurred when alpha-2-plasmin inhibitor levels measured less than 30% of normal levels. Patients with acute promyelocytic leukemia who...

Fibrinolytic cross-talk: a new mechanism for plasmin formation.

Fibrinolysis and pericellular proteolysis depend on molecular coassembly of plasminogen and its activator on cell, fibrin, or matrix surfaces. We report here the existence of a fibrinolytic cross-talk mechanism bypassing the requirement for their molecular coassembly on the same surface. First, we demonstrate that, despite impaired binding of Glu-plasminogen to the cell membrane by epsilon-amin...

Clinical Medicine Insights: Women’s Health

Menorrhagia or Heavy Menstrual Bleeding (HMB) remains a public health challenge among women in the reproductive age group. Anti-fibrinolytics such as tranexamic acid and epsilon aminocaproic acid, play an important role in the medical management of HMB, as HMB is associated with an increase in local fibrinolysis. Lysteda is a novel oral formulation of tranexamic acid which has recently been app...

The role of antifibrinolytic agents in gynecologic cancer surgery.

OBJECTIVE To compare the effects of crystalloid and colloid solutions, tranexamic acid and epsilon-aminocaproic acid on the need for allogenic blood transfusion and on coagulation and fibrinolysis parameters. METHODS We conducted the study in the Anesthesiology and Reanimation Department of Hacettepe University Medical Faculty, Ankara, Turkey between March 2004 and April 2005. The study inclu...
